Joni: RN Care Manager. Joni graduated from AB Tech Community College with an Associates Degree in Nursing in 2013. Since then she has had RN experience as a Med-Surg RN at Mission hospital and has also worked in pediatrics. Joni is a true native to this community. Her first job was here in this practice. She worked here for seven years in different roles throughout the office until she graduated nursing school. She has always loved this practice and felt that this was her home. She returned to the office in April of 2017 in the RN Care Manager role. One of her roles is managing the care of the Medicare and higher health risk population. She does so by triaging patient issues, making and tracking referrals, helping patients enroll in medication assistance programs, disease education, and serving as a direct line from the patient to the patient's provider. She also manages the care of patients who have been admitted to the hospital. When a patient is discharged, she reaches out to them to ensure their medical needs are met and that they were able to get all of their medications at discharge. Overall, her goal is to manage the care of our patients in a way that lowers their risk of admission and re-admission into the hospital. She is another tool in the tool belt of our practice to help patients with chronic conditions to improve health outcomes.
