Appointment reminders:

We offer automated reminder calls as a courtesy to our patients. These calls are made 24-48 hours prior to your scheduled appointment.

You may opt to enroll and receive text reminders. Signing up is easy!

Important Appointment Information:

We ask that all patients arrive 15 minutes prior to your scheduled appointment time. This allows us time for the registration process and time for any changes or updates to be made to your record without delaying your appointment.

We ask that patients bring in all medications -both prescription and over-the-counter - to each appointment.

Our office cannot extend credit for services provided. Payment in full is expected at the time of service. If for any reason your balance exceeds $100, additional action may need to be taken in order to resolve this balance. Non-emergent visits may be postponed or rescheduled until payment can be made.

There is a $15 minimum charge for all forms filled out or signed by providers outside of an office visit. Exact charge will be determined by the provider upon completion.

Rescheduling or Canceling appointments:

If you find you must reschedule or cancel an appointment, please notify the office at least 24 hours prior to your appointment. This allows us to offer that appointment slot to another patient and keeps our providers more accessible when you need them. Failure to give 24 hours notice may result in a missed appointment fee that is not covered by your insurance. If you call when we are closed, please leave a message on the general message voicemail to cancel your appointment.

Late Arrivals

If you arrive 15 minutes after your scheduled appointment time, we will do what we can to accommodate the situation and work you in to be seen depending on the day and the provider, you may be asked to reschedule. Please contact our office if you are running late for an appointment.

During each visit to our office, please be prepared with your most current insurance card. You will be asked to confirm your address, phone number, and insurance coverage every time you visit our office during check in. We appreciate your cooperation as we strive to provide the highest level of care from start to finish by preventing unnecessary demographic mistakes that can prevent/delay claim payment resulting in incorrect billing. This also assures that we are able to reach you with pertinent health-related information in a timely manner.
